This online Associated Entity application form is to be used where the applying entity has only natural persons (Associated Individuals) associated with it. Where the applying entity has other entities associated with it, the applying entity must complete its application using the PDF application. Applying for approval as an Associated Entity of a Commercial Raffle Organiser

All entities who are associates of a Commercial Raffle Organiser must be approved by the Victorian Gambling and Casino Control Commission (VGCCC).

Before you start you must have obtained:
    Business Credit File from either Equifax or illion
    Historical Organisational Extract from the Australian Securities and Investments Commission (ASIC)

The above documents must not be more than three months old at the time of lodgement of this application.

Other documents may be required depending upon the response provided in this application.

All documentation supplied with this application must be in digital form and attached where required.

Processing your application

This Associated Entity application cannot be submitted until all Associated Individual forms associated with it have been completed and submitted. This Associated Entity application can be ‘Saved as Draft’ at which point an email will be sent to all individuals to complete associated applications. You can view the list of associates and the status of their applications at the top of the Contact TAB.

You will receive an email notification to submit this Associated Entity form once all related associate forms have been completed.

Your application may require an Associated Individual form to be completed by each individual who is associated with the Entity that is applying to be associated with a Commercial Raffle Organiser. Only one form is required for each individual irrespective of the number of relationships the individual may have with your entity.

  1. a person listed on the Roll of Manufacturers, Suppliers and Testers; or
  2. a holder of the Monitoring Licence; or
  3. an associate, subsidiary or related body corporate of a person listed on the Roll; or
  4. an associate, subsidiary or related body corporate of a holder of the monitoring licence
