This online Associated Entity application form is to be used where the applying entity has only natural persons (Associated Individuals) associated with it. Where the applying entity has other entities associated with it, the applying entity must complete its application using the PDF application. Please refer to the VCGLR website to obtain a copy of the PDF.
Applying for approval as an Associated Entity of a Venue Operator
All entities who are associates of a Venue Operator must be approved by the Victorian Commission for Gambling and Liquor Regulation (VCGLR).
Before you start you must have obtained:
The above documents must not be more than three months old at the time of lodgement of this application.
Business Credit File from either Equifax or Dun & Bradstreet
Historical Organisational Extract from the Australian Securities and Investments Commission (ASIC)
Other documents may be required depending upon the response provided in this application.
All documentation supplied with this application must be in digital form and attached where required.
Processing your application
This Associated Entity application cannot be submitted until all Associated Individual forms associated with it have been completed and submitted. This Associated Entity application can be ‘Saved as Draft’ at which point an email will be sent to all individuals to complete associated applications. You can view the list of associates and the status of their applications at the top of the Contact TAB.
You will receive an email notification to submit this Associated Entity form once all related associate forms have been completed.
Your application may require an Associated Individual form to be completed by each individual who is associated with the Entity that is applying to be associated with a Venue Operator. Only one form is required for each individual irrespective of the number of relationships the individual may have with your entity.
No payment is required for these additional applications.
Your application may only be submitted once all Associated Individual applications have been received.
Check your eligibility
You will not be eligible to be considered for approval as an Associated Entity of a Venue Operator if you are
- a person listed on the Roll of Manufacturers, Suppliers and Testers; or
- a holder of the Monitoring Licence; or
- an associate, subsidiary or related body corporate of a person listed on the Roll; or
- an associate, subsidiary or related body corporate of a holder of the monitoring licence

False or Misleading Information
It is an offence under the Gambling Regulation Act 2003 to give information that is false or misleading. If you give false or misleading information, your application may be refused and you may be prosecuted.
Notification of changes
Once you have submitted this application and prior to determination by the VCGLR you must notify the VCGLR in writing of any changes to the information you have provided.
If approval is granted, you will be given a set of directions where you have an ongoing responsibility to notify the VCGLR of specified changes in your situation. You must notify the VCGLR in writing within 14 days of any specified change taking place.
Saving your application
Your application will be auto saved during the course of your application.
You will be able to save a partially completed application and return to your application for a maximum of thirty business days. You must supply a valid email address and mobile phone number (for security validation) to use this facility.
Associated Individual forms required to accompany this application
Any associated individuals identified in questions 2, C2 and C3 will receive an email requesting them to complete an Associated Individual form.
All requested forms must be completed and submitted before your application can be accepted by the VCGLR.
